Notification and Vaccine Voucher

With regards to the vaccination of Covid-19 infection, special measure concerning temporary vaccination on Immunization Act has been provided. As a rule, you are able to receive the vaccine for free at the local municipality where your certificate of residence is registered.

You will receive the following notification and vaccine voucher.

The contents of the notification are as follows,

◎Vaccination guide 

1 Checking your period that meets your vaccination eligibility.

Initial Covid-19 vaccination will be carried out to health care workers, senior citizens, and persons with underlying condition, etc. Check and wait for your vaccination day.

※Wait for the announcement of national government. Refer to website of the Ministry of Health, Labour and Welfare, or consultation desk of your municipality as well.
※See reverse side for details of senior citizens, and those who have underlying condition.
※Please note that you might wait for your vaccination turn.It is important to wait with peace of mind, as vaccines will be provided successively to all persons who wish to receive.

2 Searching for medical institution / venue for vaccination

Newsletters, and website of your municipality will offer you medical institutions and venues that are available to receive the vaccine.

※In case you are not able to find medical institution or venue of vaccination, contact to the municipality where you live.
※In principle, Vaccination is conducted at the municipality where you have certificate of residence (domicile) except those who are in hospital / facility. Please see the information below for vaccination venue other than your domicile.
※‘Corona Vaccine Navi’ does not accept direct vaccination appointment.

3 Making an appointment and receiving the vaccine

Contact to the medical institution / municipality that you wish to receive the vaccine.

Check body temperature at home before vaccination. If you have obvious fever, and/or you are not in good condition, contact to municipality or medical institution that you made an appointment.

※In order to raise vaccine efficacy, participants need to receive the second dose of the same vaccine after certain days of the first one.
※When the first dose is carried out, confirm the first day your second one is scheduled to be given.
※Enclosed paper vouchers in sheet include 2 doses of ‘vaccine voucher’, and ‘certificate for vaccination completion’
※Whenever you visit the vaccination venue, bring vouchers that are not separated from paper sheet.

The contens are as follows,

◎Vaccination for senior citizens

Those who will reach 65 years or older in 2021 (those born on/before April 1, 1957) is scheduled to be initial vaccination participants. Those might be divided into smaller groups with respect to vaccination timeline.

◎Those who have underlying condition are…

Those who have underlying condition are scheduled to receive the vaccine after senior citizens. Those with underlying condition are applied to any of the followings.
Any of the followings are applied to persons with underlying condition.

(1)Those who are outpatient / hospitalized due to suffering following diseases / conditions

   1  Chronic respiratory organs diseases 慢性の呼吸器の病気
   2  Chronic heart diseases (including high blood pressure) 慢性の心臓病(高血圧を含む)
   3  Chronic kidney diseases 慢性の腎臓病 
   4  Chronic liver diseases (not including fatty liver and chronic hepatitis disease)
         慢性の肝臓病(ただし、脂肪肝や慢性肝炎を除く) 
    5  Diabetes that is under treated by insulin or other medicine,
   or diabetes that is complicated with another disease.
         インスリンや飲み薬で治療中の糖尿病、又は他の病気を併発している糖尿病 
    6  Blood diseases (not including iron-deficiency anemia)
   血液の病気(ただし、鉄欠乏性貧血を除く)
       7  Diseases that decrease immunity function (including malignant tumor under treatment)
         免疫の機能が低下する病気(治療中の悪性腫瘍を含む)
       8  Those who are under treatment for declining immune function by steroid, etc.
         ステロイドなど、免疫の機能を低下させる治療を受けている
       9  Nervous / neuromuscular diseases due to immune abnormality
   免疫の異常に伴う神経疾患や神経筋疾患
 10 Conditions that physical functions are lowered due to nervous / neuromuscular diseases
      (respiratory disturbance, etc.) 神経疾患や神経筋疾患が原因で身体の機能が衰えた状態(呼吸障害等) 
    11 Chromosomal aberration 染色体異常
    12  Severe motor and intellectual disability (conditions of severe physical and intellectual disability)
           重症心身障害(重度の肢体不自由と重度の知的障害とが重複した状態)
     13  Sleep-apnea syndrome 睡眠時無呼吸症候群

(2)Those who meet BMI (30 or more) standard and considered to be obese
   BMI=weight(kg) ÷height(m)÷height(m)
   ※numerical example of BMI30  
 170cm in height and 87kg in weight, 160cm in height and 77kg in weight
   Please note that at the same period, vaccination is scheduled to be provided to those who work at elderly care facilities.


◎Receiving the vaccine other than the municipality where your certificate of residence is registered (domicile)

・Those who will receive the vaccine at a staying medical institution / residing facility.
  →Please contact to a relevant medical institution/ facility.

・Those who will receive the vaccine at a medical institute being treated for underlying condition.
  →Please contact to a relevant medical institution/facility.

・Those whose living place is different from registered domicile.
  →Participants may receive the vaccine where you live at present.

・Please check Coronavirus Navi, or contact to consultation desk of municipality where you live at present.

◎Participant’s consent is required when receiving the vaccine.

Those who are receiving medical treatment due to disease, and/or anxious about receiving the vaccine due to physical conditions, consult with family doctor and decide receiving/not receiving the vaccine.

If you have an underlying condition, vaccination at a medical institution (family doctor, etc.) is highly recommended.

If you wish to receive vaccination at a family doctor outside the city, please consult your doctor.

Vaccine cancellation standby appointment

In order to provide vaccine effectively, vaccine cancellation standby appointment is available for those who wish to receive if someone cancels due to health condition, etc.

Regular vaccination appointment is also available after signing up cancellation standby appointment.

 (Note) You will receive the call on the day when cancellation is occurred.
               You are not able to choose vaccination venue and date.

What you bring/wear on the day of vaccination

・Vaccination voucher 

・Vaccination primary questionnaire (Fill it out in advance to make the smooth vaccination process.)
    Translations of Prevaccination Screening Questionnaire for COVID-19 vaccine in multiple languages.

・ID (My Number Card, Health Insurance Certificate, driver’s license, etc.)

・Check list for those with underlying medical condition.

・Wear sleeves that rolled up easily to the shoulder (Vaccine is given at shoulder.)

For age 12-15, when they visit to a medical institution for a vaccination, let them bring a preliminary examination slip with their guardian's consent signature, and guardian(s) should accompany them.

If you are unable to receive vaccine due to fever/health problem, etc. you are required to contact to the medical institution or call the number below promptly.

Booster(third) Vaccination

To make vaccine more effective,
booster (third) vaccination is available for those who had the second vaccination basically over 8 months ago and ages 18 and up.

Regarding a booster dose of COVID-19 vaccine, mRNA vaccine (vaccine of Pfizer, Takeda/ Moderna) is used regardless what vaccine type people had in the first 2nd doses.
The dose in the Pfizer booster is the same amount as in the first two shots. But at 50 micrograms, Takeda/Moderna’s booster is half the amount used in the initial two.

According to an US study that evaluate the effectiveness and safety of using alternate vaccine  for the booster dose (different vaccine from the first 2 doses), they reported that it increased antibody titer properly, and the side effects were similar to those reported in the first round (first and second) vaccination, and concluded that there was no difference between the alternate vaccination and the allogeneic vaccination.

Who is applicable for booster (third) vaccination and vaccination schedule

People who had the second vaccination basically over 8 months ago, and ages 18 and up.

"Preliminary examination slip with vaccination ticket printed" for the third vaccination will be delivered to your home from your residence municipality

※ Different from first and second vaccination ticket, it is not a sticker type this time, and integrated with a preliminary examination slip.

"Preliminary examination slip with vaccination ticket printed" will be delivered to you when the vaccination date comes closer.

But if any of following applies to you, Tsuchiura city might not own your vaccination record, and the vaccination ticket will not be shipped to you. If you don't receive the ticket after 8 months from your second vaccination, please apply for the third vaccination ticket to Tsuchiura city.

Reissue of Vaccination Voucher

Cases for Reissue Vaccination Voucher

When you have lost your vaccination voucher with some reason, you can request to reissue it to the municipality in which you reside.

To apply, please contact to “COVID-19 Vaccination Team” (9-2, Bunkuocho, Tsuchiura City 300-0045  TEL:029-886-5302).

Cases which require voucher reissue are as below:

・a voucher was lost/ruined/ badly damaged

・your resident’s location was changed after a voucher was issued

・you didn't receive a voucher

・you have not been registered in Resident’s Record in Tsuchiura City

・you have used "Prevaccination Screening Questionnaire voucher" twice

・when the city Mayer admitted a necessity of the reissue

 

Procedure of Vaccination Voucher reissue

Please note that it will take 1-2 weeks to issue a new voucher after each procedure.

Applying by postal mail

Fill in "Vaccination voucher reissue application", and send it to  “COVID-19 Vaccination Team” (9-2, Bunkuocho, Tsuchiura City 300-0045 TEL:029-886-5302) by mail.
If there is no issue in your application, a new voucher will be sent to you by mail.

※ If you have your old voucher, please enclose it in the envelope.

Applying at the office of “COVID-19 Vaccination Team”

Fill in "Vaccination voucher reissue application", and bring it to a counter of “COVID-19 Vaccination Team” (9-2, Bunkuocho, Tsuchiura City 300-0045 TEL:029-886-5302).
If there is no issue in your application, a new voucher will be sent to you by postal mail later.

※ If you have your old voucher, please bring it to the counter.

Applying by phone

Call “COVID-19 Vaccination Team” (TEL:029-886-5302). A staff will interview you over the phone, and if there is no issue, a new voucher will be sent to you by postal mail later.

※ If you have your old voucher, please have it on hand before you call, as a staff will ask some questions, such as your vaccination status, etc.

 

For those who moved into / moved out of Tsuchiura city

For those who moved into Tsuchiura city

Please note that you can't use the voucher in Tsuchiura city which was issued outside the city.
Please submit a reissue application to “COVID-19 Vaccination Team” and receive a new one.

For those who moved out of Tsuchiura city

Please note that you can't use a voucher which was issued by Tsuchiura city in your new residence.
Bring it to your new resident city, and apply to reissue a new one. Please follow instructions of your new residential municipality.
